Uses: Treats (1) major depression; (2)
obsessive-compulsive disorder; (3) panic disorder: (4) treats
post-traumatic stress disorder; (5) treats premenstrual
dysphoric disorder.
Other (unlabeled) generally accepted uses: (1) May have a role
in treating obesity-rat studies have shown a decrease in
eating that depends on the dose that is taken, and studies in
humans are being conducted; (2) may help some kinds of sexual
problems (premature ejaculation); (3) helps clozapine-caused
(induced) obsessive-compulsive disorder; (4) eases symptoms in
premenstrual dysphoric syndrome (PMDS); (5) can ease the
feeling of loss of breath in mild to severe obstructive lung
disease; (6) may have a role in depression after a heart
attack, both in treating the depression and in heart rate
variability.
How This Drug Works: This drug relieves depression by
slowly restoring to normal levels a specific constituent of
brain tissue (serotonin) that transmits nerve impulses.
Available Dosage Forms and Strengths
Tablets - 25 mg, 50 mg, 100 mg
Recommended Dose Ranges (Actual dose and schedule must
be determined for each patient individually.)
Infants and Children: In children 6 to 12 years old: 25
mg once daily has been
approved to treat obsessive-compulsive disorder.
12 to 60 Years of Age: Depression: Initially 50 mg once
daily, taken in the morning or evening. The dose is then
slowly increased, as needed and tolerated, in increments of 50
mg at intervals of 1 week. The total daily dose should
not exceed 200 mg.
Panic Disorder: Dosing is started at 25 mg a day
(morning or evening). After seven days, the dose can be
increased to 50 mg once daily. If needed and tolerated, the
dose can then be increased (at 7 -day intervals) to a 200 mg
per day maximum.
PTSD Similar to dosing for depression.
Obsessive-compulsive disorder: Same as dosing in
depression.
Over 60 Years of Age: Dose amounts are the same as 12
to 60 years of acre unless liver function is compromised.
Because the drug is removed some 40% more slowly than in
younger patients, any changes in dose should be made at 2-3
week intervals.
Conditions Requiring Dosing Adjustments
Liver Function: Drug is a rare cause of liver damage.
Patients with liver disease should be watched closely and
lower doses used.
Kidney Function: The role o( the kidneys is minimal and
dosing adjustments are not thought to be needed.
Dosing Instructions: The tablet may be crushed and is
best taken with food to enhance absorption, but it may be
taken at any time with or without food The oral liquid form
(concentrate) has 20 mg in each m1. Use the calibrated dropper
to get the right dose. Just before you are going to take it,
the dose can be mixed with 4 ounces of water, orange juice,
ginger ale, lemonade, or other suitable liquid (talk to your
pharmacist). It is okay if a haze appears once the concentrate
has been added to the liquid. Taking this medicine regularly
is important. If you forget a dose: Skip the missed dose and
take the next dose right on schedule. DO NOT double doses.
